Key Responsibilities
- Review prescriptions thoroughly to verify legal compliance, clinical appropriateness, dosage accuracy, treatment duration, and patient safety before dispensing medicines.
- Evaluate prescriptions for potential drug interactions, contraindications, allergies, duplicate therapies, and possible medication-related risks.
- Dispense prescription medicines accurately while strictly observing nationally approved pharmaceutical standards and Good Dispensing Practices.
- Prepare individualized extemporaneous pharmaceutical formulations whenever commercially manufactured products are unavailable or clinically unsuitable.
- Counsel patients comprehensively on medicine administration, dosage schedules, storage conditions, expected therapeutic outcomes, precautions, and possible side effects.
- Promote medication adherence by educating patients on the importance of completing prescribed treatment regimens.
- Recommend suitable over-the-counter medicines for minor illnesses within professional practice guidelines and refer patients requiring further medical evaluation.
- Participate actively in multidisciplinary ward rounds to provide pharmaceutical recommendations that improve patient treatment plans.
- Monitor therapeutic effectiveness and communicate medicine-related concerns to clinicians to support evidence-based decision-making.
- Identify pharmaceutical care challenges affecting healthcare delivery and recommend practical improvement strategies.
- Maintain accurate pharmaceutical documentation, dispensing records, prescription registers, and daily activity logs.
- Monitor medicine inventory levels to minimize shortages, overstocking, wastage, and expiry of pharmaceutical products.
- Ensure medicines are stored under appropriate environmental conditions while complying with quality assurance standards.
- Participate in forecasting pharmaceutical requirements and supporting medicine procurement planning.
- Verify deliveries of pharmaceutical products against approved procurement specifications and inventory records.
- Report suspected adverse drug reactions and medication safety incidents through appropriate pharmacovigilance systems.
- Support implementation of antimicrobial stewardship initiatives that promote responsible medicine use.
- Participate in medicine utilization reviews and clinical audits to improve pharmaceutical service quality.
- Educate patients, caregivers, and healthcare workers on safe medicine practices and disease prevention strategies.
- Uphold confidentiality while handling patient information and pharmaceutical records.
- Maintain compliance with Pharmacy and Poisons Board regulations and county healthcare policies.
- Utilize electronic inventory management systems and pharmacy information systems accurately.
- Participate in departmental meetings, quality improvement activities, and continuous professional development programs.
- Assist during health outreach campaigns, wellness initiatives, and public health education activities when required.
- Mentor pharmacy interns, students, or junior staff where assigned.
- Collaborate effectively with healthcare professionals to promote integrated patient care.
- Ensure safe disposal of expired, damaged, or obsolete pharmaceutical products according to approved procedures.
- Contribute to maintaining a clean, organized, and compliant pharmacy environment.
- Support emergency pharmaceutical services during public health emergencies whenever required.
- Perform any additional pharmacy-related responsibilities assigned by authorized supervisors in support of county healthcare objectives.
Qualifications & Requirements
- Bachelor of Pharmacy (B.Pharm) degree from a recognized institution approved by the Pharmacy and Poisons Board.
- Successful completion of a one
- year pharmacy internship at a recognized healthcare institution.
- Registration Certificate issued by the Pharmacy and Poisons Board.
- Valid Practicing License issued by the Pharmacy and Poisons Board.
- Certificate in Computer Application Skills from a recognized institution.
